Health Connect America’s mission is dedicated to providing quality and affordable services to individuals and their family members. Doing so allows them the space to achieve the highest level of potential and may reside in a safe, positive living environment. We accomplish this by living out our mission and values in all that we do. We provide face-to-face support, counseling, and care coordination with the individual, family and significant others, as well as behavioral, emotional and educational support. Ideally, the support and services will be provided in the client’s home or community.
